Full Time Nurse Navigator 2 to work at the UHealth Medical campus
in Miami, Fl.Core Job SummaryThe Nurse Navigator 2 Educates
patients and their families about their diagnosis and assures
timely scheduling of the first appointment, and coordination of
care after, and supports the patient throughout the care continuum.
The Nurse Navigator 2 liaises between patients, families,
caregivers, and the multidisciplinary care team to provide
appropriate information and resources during the diagnostic
evaluation. The incumbent demonstrates a commitment to quality
patient care, implements creative and innovative ways to meet the
diverse needs of the patients, and ensures best practices.Core Job
Functions
- Triages new patient appointments to the appropriate provider(s)
and assures timely scheduling of initial appointments.
- Reviews outside medical records for appropriate
scheduling.
- Assess barriers to care and refers to support services, local,
and national organizations when needed.
- Educates on the treatment plan for patients based on
diagnosis.
- Supports patients throughout the care continuum.
- Counsels individuals and patients on positive health
practices.
- Collaborates with a multidisciplinary team of experts to
outline the best treatment for patients.
- Performs holistic evaluation of specialty population, making
use of enhanced proven techniques and procedures to achieve better
results.
- Implements the improvement of patient care, and healthcare
policies and resources.
- Mentors other healthcare professionals by functioning as a
preceptor or coordinating preceptors for visiting professionals,
students, new graduates, and orienteers.
- Maintains professional knowledge by affiliating with
professional and technical organizations, and participating in
applicable continuing education programs, conferences, seminars,
and workshops.
- Adheres to University and unit-level policies and procedures
and safeguards University assets.This list of duties and

relevant fieldCertification and Licensing:Valid State of Florida
Registered Nurse (RN) licenseExperience:Minimum 3 Years Of Relevant
Work ExperienceKnowledge, Skills and Attitudes:
- Ability to maintain effective interpersonal relationships.
- Ability to communicate effectively in both oral and written
form.
- Skill in collecting, organizing and analyzing data.
- Proficiency in computer software (i.e., Microsoft
Office)Department Specific Functions
- Patient Navigation & Education
- Serve as the primary point of contact for patients navigating
cancer screening services.
- Provide culturally sensitive education on cancer screening
guidelines, procedures, and follow-up care.
- Assist patients in overcoming barriers to screening (e.g.,
transportation, language, health literacy).
- Ensure timely scheduling of screenings, diagnostic follow-ups,
and referrals.
- Process Improvement & Quality Assurance
- Identify gaps in current screening workflows and recommend
evidence-based improvements.
- Support cancer screening initiatives in alignment with
institutional and public health goals.
- Coordinate outreach campaigns and events to increase screening
uptake.
- Track project timelines, deliverables, and performance
metrics.
- Collaborate with internal departments (e.g., IT, quality,
communications) to implement and evaluate screening workflows.
- Monitor key performance indicators (e.g., screening rates,
follow-up timeliness) and support action plans for
improvement.
- Maintain accurate documentation and reporting in compliance
with organizational standards.
- Data and Reporting
- Use electronic health records and registries to track patient
screening status and outcomes.
- Prepare reports on screening metrics and present findings to
leadership or multidisciplinary teams.
- Assist in grant reporting or research activities related to
screening outcomes, if applicable.
- Collaboration & Training
- Provide training and education to clinical and administrative
staff on screening protocols and navigation processes.
- Support community partnerships and coalitions to expand access
to screening services.
- Represent the screening program in internal and external
meetings or forums.Knowledge, Skills and Abilities:
